Treatment of bladder exstrophy

Andrology – EURODOCTOR.RU – 2007

Bladder exstrophy is a congenital anomaly of the genitourinary system. Very often schistocystis combined with such a malformation of urethra as epispadias. This is perhaps the most severe congenital malformation in urology, in which there is no front wall of the bladder and the corresponding anterior abdominal wall (tummy).

The incidence of bladder exstrophy

30,000 – 50,000 newborns found a case of exstrophy of the bladder. The ratio of boys – girls is 2:1. at the same time. The risk of having a second child with bladder exstrophy, if the pathology was a first child is 1:100, and 1:70 in the presence of exstrophy of one of the parents.

Causes of bladder exstrophy

The exact causes of this anomaly is not installed. However, we know that its development is influenced by factors such as genetic predisposition, and environmental insults.

Manifestations of bladder exstrophy

As stated above, this anomaly is most often combined with epispadias, urethral malformation, in which he has no front wall. It is noted the splitting of the urethra and the head. Open out the bladder and deformed genitals are visible in the first moment after birth, All the urine flows out. In addition, these patients have bone and divergence symphysis pubis. In this regard, they have observed so-called "duck" gait.
Care of patients with bladder exstrophy is difficult. These children are deprived of the opportunity to play with peers. Constant contact with the mucous membrane of the external environment, as well as malformations of the upper urinary tract contributes to the rapid emergence of pyelonephritis – an inflammation of the kidneys.

Treatment of bladder exstrophy

Currently, urologists consider the most appropriate age for corrective surgery on the newborn period up to 2 – 3 years.
There are three groups of surgical interventions:

  • Reconstructive and plastic surgery. To the plastic front wall of the bladder and the abdominal wall due to local tissues or the use of various plastic materials.
  • Transplantation of isolated ureter or with vesico triangle in the rectum
  • Creating an isolated bladder.
