Antihypertensive effect of Losartan

CARDIOLOGY -2008

Losartan (Cozaar) – is a product of angiotensin receptor blockers-II (ARB). Angiotensin – a substance that is formed from renin, synthesized in the kidney. the effect of angiotensin is to reduce the blood vessels, resulting in increased blood pressure. Angiotensin exerts its effect through the so-called receptors, which are mainly concentrated in the walls of arteries. Angiotensin is attached to this receptor, resulting in lumen narrowing of the arteries.

Losartan is attached to the receptor blockers, and angiotensin no longer has its effect. This leads to a reduction of high blood pressure.

Application: as drugs, ACE inhibitors, losartan is used to treat hypertension, heart failure and diabetic nephropathy. It may be used alone or in combination with other drugs.

Dosage: usually assigned to losartan 50 mg per day for adults. The maximum dose is 100 mg. The daily dose at the same time can be divided into several parts. It can be taken regardless of meals.

Interactions with other drugs: the simultaneous administration of drugs, inhibitors of cytochrome P450 (eg, fluconazole, Nizoral) may experience reduced activity of losartan, which must be considered.

Losartan may be accompanied by a significant increase in the level of potassium in the blood, resulting in severe arrhythmia may occur. It is therefore not recommended while taking losartan and potassium-sparing diuretics (triamterene, amiloride).

Antihypertensive effect of losartan may be reduced when combined with nonsteroidal anti-inflammatory drugs (aspirin, naproxen, ibuprofen, indomethacin, etc.).

Pregnancy and breast-feeding: not recommended to use the drug during pregnancy, especially during the second and third trimesters, as it could have an adverse effect on the fetus. Therefore, if the patient was found pregnant, the drug should be discontinued.

Is not known whether losartan or its metabolites with human milk, but in rats it is excreted. So do not take medication during breastfeeding.

Side effects. In clinical studies the incidence of adverse effects of losartan was similar to that of placebo. Among the side effects were observed: diarrhea, muscle cramps, dizziness, insomnia, and nasal congestion. In addition, there may be a cough, increased levels of potassium in the blood and angioedema. Losartan may have a negative effect on renal function in some patients, it is not recommended to apply to persons with bilateral renal artery stenosis.
